Information From Your School Nurses

Welcome!

We are so excited to have a happy and healthy school year. Please help us by making sure the nurses have your student’s health information.

Complete Immunization Records-please turn in any updated records or exemption paperwork as soon as possible. Every student in Colorado is REQUIRED to have documentation related to immunizations provided to the school. 

Emergency Medications and Paperwork-all emergency medications (ie: inhalers, epi pens, seizure medications) and updated paperwork MUST be turned in as soon as possible. Students will not be allowed to attend field trips if we have not received these items. Please contact the school nurse to discuss any medical conditions your student has.

Over the Counter Medication Form-if you would like your student to be able to receive some over the counter medications please have your healthcare provider complete an Over the Counter Medication Form. These must be signed by the healthcare provider and the parent. 

Daily Medications and Paperwork-if your student requires medication to be given daily while at school please provide a medication supply that can be left in the nurse’s office. Your provider will need to complete paperwork so this medication can be given daily to your student. Please contact the school nurse to discuss any medical conditions your student has. 

If your student is not well please use the How Sick is Too Sick document to determine if they should attend school. 

If you have questions or concerns about your student’s health or wellness please contact the school nurse. If you need assistance obtaining medical appointments, medication, paperwork or forms the nurses are available to help. There are also many healthcare resources available through the Grand County Rural Health Network.
